AC Tune Up | Energy Star Appliance Rebate | HVAC Equipment Installation Forms

The subject Installation Form from Elk River Municipal Utilities can be downloaded from this site, or mailed to you if you request.  Please complete one of these forms for each new air conditioner or air source heat pump you install in our service area.  One copy (white) should stay with the homeowner, two copies (yellow and pink) should be returned to us, and a copy (gold) should be retained in your files.  Once this form, plus a rebate application (found in our Energy Star Appliance Rebate brochure) and a sales receipt for the purchase of same is returned to us we verify the SEER and do an on-site inspection prior to processing the rebate.  That process does not begin until we have the Equipment Installation Form, Rebate Application Form, and sales receipt on file!  If the system was installed too early or too late in the year to do the performance tests, please inform the homeowner that there will be a delay in receiving a rebate from us until the weather warms up and these tests can be performed by your firm.     

We also offer $100 ECM blower motor rebates for new furnaces if they are so equipped, and $50 ECM blower motor rebates for retrofitting existing furnaces with one.  Please fill out one of these Equipment Installation Forms for that new or retrofitted furnace. 

It is extremely helpful if you include a copy of the AHRI Certificate Of Product Ratings!


 The AHRI certified reference number on that certificate must match the info on the Equipment Installation Form.
